Diabetes Specialist Nursing Team – Structured Education

What is this service?

Structured education to all those diagnosed with type 2 diabetes. Courses are delivered at a range of community venues around the county, plus online.

Diagnosis of type 2 diabetes:

• Can access the following: X-PERT digital App
• X-PERT Diabetes 6-week course either face to face or virtual

Diagnosis of type 1 diabetes:

• Patients with type 1 diabetes have open access to the Community Diabetes Specialist Nursing Service, and also for structured education (DAFNE course).

For any with pre-diabetes (HbA1c between 42 to 27mmol/mol) GP must refer to the Diabetes Prevention Programme. https://www.lwtcsupport.co.uk/about
